Stoma HuidCheck

De Stoma HuidCheck is een gratis app die je helpt de huid rond je stoma goed in de gaten te houden. Op een heel makkelijke, snelle en veilige manier. Gewoon via je smartphone waar en wanneer het jou uitkomt. Een probleem met de huid kan langzaam optreden en wordt vaak gezien als iets wat erbij hoort. Dit is echt niet het geval en hoe eerder je erbij bent, hoe beter. Want er is veel aan eventuele huidproblemen te doen.

Open de Stoma HuidCheck-app en volg de stappen op je scherm. Er worden een aantal gegevens van je gevraagd, zoals wat voor stoma je hebt en sinds wanneer. Deze informatie is nodig om je goed te kunnen helpen. Het belangrijkste onderdeel van de app is dat we je vragen een foto van je stoma en de huid er omheen te maken. De app helpt je dit goed in beeld te brengen. Afhankelijk van de plaats van je stoma kun je deze foto zelf nemen of heb je hulp van iemand anders nodig.

De stomaverpleegkundigen van Hollister en Dansac bekijken de foto met de aanvullende gegevens en checken of er reden is om een gespecialiseerde verpleegkundige te raadplegen. Deze check kan twee werkdagen duren en je krijgt hier vanzelf bericht van. Als we je aanraden om naar de huid rondom je stoma te laten kijken, verwijzen we je altijd door naar je eigen stomaverpleegkundige. En als je die niet hebt, helpen we je iemand te vinden.

DEEL WAT JE ZIET EN VOELT

Deel de kenmerken van de huid rond de stoma door deze te vergelijken met de huid aan de andere kant van jouw buik.

DEEL AANVULLENDE INFORMATIE

Geef ons een aanvullende omschrijving die ons helpt bij een goede check. Je kunt hierbij onder andere denken aan: wanneer is het ontstaan, hoe lang is het aanwezig, is het stabiel of verergert het, is er een aanwijsbare oorzaak etc.

MAAK EEN FOTO

Maak een foto waarop de huid rond de stoma goed zichtbaar is. Zijn er aanvullende foto’s nodig voor een goede check, dan zullen de stomaverpleegkundigen van Hollister en Dansac hier om vragen.

DEEL AANVULLENDE GEGEVENS EN VERSTUUR

Alleen de eerste keer word je gevraagd enkele aanvullende gegevens te delen die wij nodig hebben om de huid rond jouw stoma goed te checken en tips te geven. Daarnaast dien je akkoord te gaan met de privacy verklaring en gebruiksvoorwaarden.

STOMAVERPLEEGKUNDIGE & TIPS

De stomaverpleegkundigen van Hollister en Dansac checken de kenmerken, omschrijving en foto en geven tips binnen de app. Als we je aanraden om naar de huid rondom je stoma te laten kijken, verwijzen we je door naar je stomaverpleegkundige. En als je die niet hebt, helpen we je op weg.

ALS JOUW CHECK IS AFGEROND

Als jouw check is afgerond kun je deze afsluiten. Afgeronde Stoma HuidChecks blijven beschikbaar in jouw app. Als je de check hebt afgesloten kun je een nieuwe check openen.

CHECK JE HUID REGELMATIG

Een probleem met de huid kan langzaam optreden en wordt vaak gezien als iets wat erbij hoort. Dit is echt niet het geval en hoe eerder je erbij bent, hoe beter. Want er is veel aan eventuele huidproblemen te doen.

Door onze ruime ervaring en onderzoeken weten wij hoe belangrijk de huid rond je stoma is, het is misschien wel het belangrijkste stukje huid dat je hebt. Zorg hier goed voor!

The Stoma Skin Check is a free app that helps you keep a close eye on the skin around your stoma. In a very easy, fast and safe way. Just via your smartphone when and where it suits you. A problem with the skin can occur slowly and is often seen as part of it. This really isn't the case and the sooner you get there the better. Because there is a lot to do about any skin problems.

Open the Stoma SkinCheck app and follow the steps on your screen. You will be asked for a number of details, such as what kind of ostomy you have and since when. This information is necessary to help you properly. The most important part of the app is that we ask you to take a picture of your stoma and the skin around it. The app helps you to visualize this well. Depending on the location of your ostomy, you can take this photo yourself or need help from someone else.

The stoma nurses of Hollister and Dansac look at the photo with the additional information and check whether there is reason to consult a specialized nurse. This check can take two working days and you will automatically be notified. If we advise you to have a look at the skin around your ostomy, we will always refer you to your own ostomy nurse. And if you don't have one, we'll help you find someone.

TAKE A PICTURE

Take a picture showing the skin around the stoma. If additional photos are required for a proper check, the stoma nurses from Hollister and Dansac will ask for this.

PART ADDITIONAL INFORMATION AND SEND

Only the first time you will be asked to share some additional information that we need to check the skin around your stoma and give tips. In addition, you must agree to the privacy statement and terms of use.

STOMA NURSE & TIPS

The stoma nurses from Hollister and Dansac check the characteristics, description and photo and provide tips within the app. If we recommend that you have a look at the skin around your ostomy, we will refer you to your ostomy nurse. And if you don't have one, we'll help you get started.

IF YOUR CHECK IS COMPLETED

When your check is complete you can close it. Completed Ostomy Skin Checks remain available in your app. When you have completed the check you can open a new check.
